being in nursing/doctor professions generally means you dont get time off and your work hours all depend on the need of the facility you are employed at. and considering how at least in the US nurses are treated very poorly if they are injured and cant work anymore it is not exactly a profession people get into as often anymore. Though yes most emergency service workers can be on the clock for extreme periods of time.
another profession that has some, interesting hours is underwater welders. generally they are on for 6 months straight then off 6 months. *basically even when sleeping they are getting paid so long as they are on the job site and starting pay is around $100-$120 USD/hr*. though you need to be a certified scuba diver first.
